Your current dose and when it started, including any titration steps you have moved through. The date matters as much as the amount.
Side effects as they actually happened: what you felt, roughly when after a dose, how long it lasted, and whether anything made it better or worse. Patterns are more useful than a single dramatic day.
Practical changes: appetite shifts, energy, sleep, clothes fitting differently, and anything you stopped or started doing because of how the medication feels.
Questions as they occur to you between visits, not the three you can remember in the waiting room.

What should make me call you before my next scheduled visit? This one question gives you a safety net and a clear threshold.
What does the next step look like if this dose keeps working — and if it doesn't? Knowing both paths in advance makes waits less anxious.
How should this medication fit around the other things I take, including supplements and anything new another clinician prescribes?
If you track with a partner
Many people start GLP-1s alongside a partner or family member on the same journey. Comparing timelines is genuinely useful — what week the appetite changed, how titration felt — but remember that two people on the same medication can have very different experiences. Your notes are yours; your prescriber's guidance is individual.
